FantaCo's Chronicles Series #2 is a more accurate title for the publication referred to as Fantastic Four Chronicles #1 in your checklist.
Regarding the issue's contents - As described at www.comics.org, in addition to the 1-page illo of the Fantastic Four by Perez, the cover of Fantastic Four #191 by Perez and Joe Sinnott accompanies the 3-page interview with Len Wein and Marv Wolfman.
There's also a house ad within for Gates of Eden #1 & #2 from FantaCo Enterprises. As seen in the 'Errata' section of your site, a (different?) ad in Comics Scene (Vol.1) #6 listed George Perez as being a contributor for Gates of Eden #2, while, in fact, he wasn't.

Elson's Presents was a series that repackaged January 1981-dated releases from DC Comics.
Elson's Presents #2 includes a reprint of Justice League of America #186, while Elson's Presents #3 includes a reprint of New Teen Titans #3.
