I went straight to the source for all things Legion, our Publisher and former LEGION scribe Paul Levitz. Here he is...
"It's ADVENTURE COMICS #310... Mr. Mxyzptlk's descendant (though we don't know that until the end) kills off the whole team until made to say his name backwards, sending him back to the 5th dimension and undoing his magical deeds. First LSH I read, coverless in a barbershop in Barryville, NY, not that it matters... Reprinted in one of the ADVENTURE DIGEST issues in the late '70s, if memory serves, and in LEGION OF SUPER-HEROES ARCHIVES Volume 2."
