I got this information and scan from their latest catalogue (issue #8).
It's official: Dick Giordano is doing the Head Sketches instead of George Pérez (announced originally with Crisis #1 ME Remarked). Those who bought or ordered your copies of the rare Head Sketches, contact Dynamic Forces and get your money back, if you're unhappy about the wrongful announcement that Pérez was doing the head sketches. I would've bought both heads, if not for the warning thanks toAndy Mangels.
Also, I am assuming that this comic book is released. I can't tell, because their website is really out-dated.

Just wanted to send out a warning to buyers and/or shop owners that the
Dynamic Forces "Crisis ReMarked Edition" on page 238 of the new Diamond
catalog is NOT Perez ReMarked, despite the misleading copy.
It states, "Signed by writer Marv Wolfman and signed/re-marked by the man who
drew the deaths of both Barry Allen and Supergirl. . . George Perez (with
Dick Giordano) signing and re-marking! This Millennium Edition reprint edition
is available with a Supergirl head sketch or a Flash head sketch! Extremely
limited!" The price for either is $69.95.
I spoke with George last Wednesday, and he stated that Dynamic Forces asked
him to do the sketches, but that he did not have time. He also stated that
he did not know WHO was doing the sketches. We both surmised that it might
by Dick Giordano, who has a lot more time these days.
Hopefully those of you who ordered this with the belief it was George can get
a refund/cancellation on your order from the store or from Diamond.
Sorry to be the bearer of bad news.

Dynamic Forces, the company that brought you such memorable items such
as the Jae Lee alternate cover to 'Ant Man's Big Christmas' and more
alternate covers to 'Ian Churchill's Lionheart' than there's been
actual issues brings two more issues into the fold.... that will make
Perez fans drool.
